Charting

0730 – Assumed care of patient.  Reported pain level of 3/10 with relief received from ordered pain medications.  Patient expressed concern that they, “were being stalked”.  Reassured patient they were not being stalked.  CQ, SVN

0800 – Patient activated call light.  Responded to patient who said, “I just saw a strange guy dressed in a Halloween costume walk past my door…”  Reassured patient they are not being stalked.  CQ, SVN

0900 – Man in black robe, holding a scythe with his bony fingers, stopped by nurses station asking to see patient.  Patient has requested no visitors, so man was turned away.  Man said, in a gravely voice, “I’ll be back later…”  Gave ordered Ativan to patient.  CQ, SVN

1030 – Patient reports they feel a “cold chill”  Provided extra blanket.  CQ, SVN

1120 – Patient now exhibiting Cheyne-Stokes respirations.  Man in black robe has returned.  CQ, SVN

1200 – Man in black robe has returned to nurses station demanding to see patient now, and that it was of utmost importance.  Turned man in black robe away, again.  CQ, SVN

1330 – Patient not dead yet.  Reports that they “feel much better now.”  Man in black robe has left the facility.  CQ, SVN
